Paw Pals Pet Dog Training Class Registration Form

• I am the actual owner of this dog, and to the best of my knowledge and belief this dog is not a hazard to persons or other dogs.

• I understand that a current rabies vaccination is required to enroll my dog in this class (Puppy Kindergarten is exempt) and have provided a copy of the certificate to the Instructor.

• I understand that it is my responsibility to ensure my dog is current on all other vaccinations and heartworm preventative, and failure to provide these may place my dog at higher risk to infectious parasites or diseases.

• I understand that while all reasonable safety measures are taken, when many dogs are brought together in a class situation there is risk involved. I accept this risk and accept full responsibility for the actions of my dog before, during, and after class.

** All dogs should wear a flat or martingale collar, and a 6 foot leash. No chain collars or leashes, or flexi-leads in class please. Thanks! **

Class size is limited and payment in full is required to secure your registration. If for any reason a course is canceled, clients will be notified by noon on the day the course is to start. Payment online at the time of registration is appreciated. To make other arrangements please contact Paw Pals at 719-684-5789.

Payments for Puppy Kindergarten at the Yorkshire location can be made directly to Yorkshire Veterinary Clinic at 719-598-3192 or online. The Puppy Kindergarten held at the Dominion Way location and all other courses are to be paid to Paw Pals Pet Dog Training by online payment or by calling 719-684-5789.
